Church Statement of Faith agreement
This form states Modgnik's Statement of Faith. Each church group leader must sign the statement there for agreeing with Modngik's stance. Signing this agreement, also indicates that the church will not go against the statement while at a Modgnik Retreat.
Waiver and Release forms
Each Church is required to follow their individual church policies with what their waiver and release forms state. Please make to have these on hand at every retreat for proper emergency procedures.
Each participant is also required to fill out the BonClarken waiver room as well.
